This release grants CCO the right to use your testimonial on our website. Please read the release. If you agree to the terms, please sign the release and send it to us by fax or mail, using the contact information below.

You may email your story to Lisa Gladstone lisa@coastalobesity.com. We would also like a photo of you for the website (preferably before and after shots). We prefer to receive photos via email, but we can accept photos through the mail as well.
